- Current CPR and First Aid certification (required)
- Current TB records showing Negative for TB (required)
- Valid driver’s license (required)
- Photocopy of Security Card (required)
- Current proof of auto insurance
- Reliable transportation
- Certification/license in the practice of Nursing (RN, LPN OR CNA) RE (required)
- PCA Competency test score report (is for applicants without Certified Nursing Assistant (CNA) certification)
- Satisfactory criminal background check (required)
- Federal fingerprinting requirements (required)
- Current Physical
- Possible random drug testing
