When you take an assessment need or contact us, our care coordinators will assist you in addressing your questions and concerns.
For further assessment, we will send our professional staff nurse to meet with you one-on-one to fully assess your needs based on your current health and personal needs. The objective is to review your personal needs with you and your family, help you identify needs, and propose a viable solution.
At the same time, we will let you know how we can work together with you or your family to ensure that proper and quality care is given. This enables us to create and maintain a care plan program that works and encourages each client’s unique and special needs, independence, and dignity.
